Hey! I figured I'd make an account here since this place seems way ahead of everywhere else I've looked when it comes to advice. Most places seem to give nothing but generic advice which I've already tried.

Around age 13 I discovered masturbation and became very fascinated with nude girls. In other words, my libido developed normally like with most boys this age. However, it only took me a year or two before my libido crashed. Suddenly, looking at naked women no longer excited me like it used to. Back then I was mostly terrified of turning out gay, but eventually I'd get over it and stop thinking about it. I still occasionally watched porn and masturbated, but it was more due to habit and boredom than lust. I don't remember exactly, but I think I had pretty severe ED at this point.

Fast forward a few years, I'm now in my early 20s. Up until this point, everything has remained the same, but suddenly out of nowhere I get this huge increase in libido. My health improves a lot in general (I was depressed and had social anxiety earlier), and for the first time in a long time I feel motivated to do something with my life. Especially I want to find a partner, since I've never had one. Anyway, this was a huge turning point in my life.

Unfortunately, this didn't really last very long. My libido began to slowly fade away after a few months. Desperately I tried everything to keep my libido high, but it just kept declining. Today, a few years later, it's pretty much non-existant. I never managed to find a partner during this time, I was too occupied with fixing my libido. So I don't really know what would happen in a sexual encounter, I can only guess.

Anyway, my sympoms:

Non-existent desire for sex/masturbation. Did noFap for 120 days recently (only broke it to see what would happen). Was the easiest thing in the world. Occasionally I might feel some desire for sex, but it never lasts very long. I also sometimes feel disgusted by the thought of sex.

Probably ED. I can only get hard by (sufficient) physical stimulation. If I stop for a moment, I become soft. Also, sometimes I seem to go soft anyway.

Probably premature ejaculation. Okay I can't know this for sure since I've never had sex, but I ejaculate fairly quickly given enough stimulation when masturbating. With less stimulation it takes longer, but as mentioned earlier I'll just lose my erection instead.


Other info:

I'm fairly healthy in other ways. I'm lean enough to have visible abs, I go to the gym several days a week. My diet is mostly meat, dairy and starch (mostly white rice, sometimes potatoes), and sometimes fruit. Right now the only supplement I take is vitamin D (5000 IU a day or so), but I've tried many of them in the past. Tried maca since many people seem to benefit from it, but it had no noticeable effect on my libido. My sleep is fine I think, I sleep enough and I can fall asleep/wake up fairly easily. I am not stressed at all.

I haven't had all my hormones tested in a while, but I did test my testosterone levels recently:
Total testosterone: 14 nmol/L
SHBG: 44 nmol/L

14 nmol/L is "okay" I think, although you'd expect something higher for someone in good shape in their 20s. However, combine that with fairly high SHBG, and my free testosterone might be too low. What do you think, does these numbers justify trying TRT?

I appreciate any advice here as I'm getting kinda desperate. Thank you!

Did you take any antidepressants kind of drugs or any hard drugs prior to that libido issue?

I've never taken any SSRI, or really any other drug for that matter (aside from anti-histamines and aspirin). The only thing I can think of that might have caused a hormonal issue is that I had surgery to both my testicles (to fix hydrocele) around the same age my libido issues started, but that could've been a coincidence.

Antihistamines like Cyproheptadine also affect on Serotonin system. I think better doing a full sex hormone checkup and start from there. Then Physical test for groin area and eliminate those probable causes.

I've used anti-histamines very sparsely in my life, it's mostly when I've had an allergy flareup, which doesn't happen very often anymore after I improved my diet. There was one time I used a much higher dose of anti-histamines just to see how it would affect me, but I couldn't really tell any difference.

I have a doctors appointment in a week, I'll ask for a full hormone checkup. I've done it before, and nothing looked out of the ordinary, but that was a while ago.

What meats do you eat?

You probably have high prolactin which will end up with this kind of problem. I would immediately test that and go from there.

Beef, pork and chicken. Right now it's mostly pork because it's cheaper, used to be mostly beef.

Elevated prolactin will lower desire, libido. Anything sexual related will be worse. Refractory periods between orgasms will be very long. Long refractory periods are the basis of POIS, and usually high prolactin is the primary cause.

whats other symptoms of high prolactin?

Worth mentioning is that I used to have pretty severe POIS. I'd feel very depressed for a day afterwards. One day it just went away, and never had it again since. This happened shortly after I had that spike in libido. Maybe I have an issue with prolactin, even after my POIS is gone, but what could cause this?

I have the same story. I had one summer at 22yo where I had a normal libido, and never again since. Sometimes I wish it had never happened so I wouldn't have spent almost a decade chasing after getting that feeling back.

My best guess is high prolactin. I have had above range prolactin on a blood test. I don't know what causes it or cures it. I've tried almost everything. I might try cabergoline as a last ditch effort even though it's a bit dangerous, after which I'm giving up on the matter altogether.

Standard recommendations for you: sunlight, carbs, easy to digest food, lifting/squats, etc.

You could have problems making enough dopamine. Vitamin C potentiates dopamine's inhibition of prolactin. Also nutrients required include BH4 (for phenylalanine -> tyrosine conversion, and tyrosine -> L-dopa conversion).

BH4 is made from B-vitamins/folate and methylation.

To make L-dopa you need iron and BH4. Hence why vitamin C is helpful, as it can help recycle BH4 and is vital for iron metabolism.

L-dopa to dopamine depends on P5P. If you have never tried high doses of P5P, you can try that and see if it helps. Otherwise if you have tried all these things I suggest you get treated medically for it with something prescribed for you. Dopamine is one of the most important neurotransmitters for quality of life, not just for sexual issues (still is a major part of it).

I don't know how high your prolactin is, but prolactinoma is technically a possibility.

Also unlikely, but manganese neurotoxicity will cause high prolactin:
