Why do my nipples get firm whenever i massage or squeeze my testicles?

Firm nipples are usually an indication of arousal (especially when sexual organs are touched/played with); this causes an activation of the sympathetic nervous system. Anything that stimulates the SNS will cause contraction of the nipples (such as cold, heat stress, fear, anxiety, etc.
So either it's stimulating or it could just be a reflex as the two are connected by the nervous system.

Yeah, no, even direct stimulation to the nipple can temporarily tighten it, but that promotes the release of prolactin, instead of testosterone. So the tightening of the nipple isn't a marker for testosterone.
