Free testosterone is normal but total testosterone is low. Can any of these lab results explain a complete absence of libido?

ESTRADIOL,ULTRASENSITIVE, LC/MS: 16 pg/mL
SHBG 13: nmol/L
Testosterone Total: 241 ng/dL
Testosterone Free: 62.2 pg/mL
PSA: 0.62 ng/mL
Dihydrotesterone: 16 ng/DL
T3 Uptake: 33%
T4: 5.6 mcg/DL
Free T4 Index: 1.8
TSH: 2.57

I should have gotten LH and prolactin. Next time I will. Any other labs that I should get that I am missing here?

Dhea, Pregnenolone, Vitamin d3 and cortisol could be good labs to get I think. Reverse t3 would give a fuller perspective on thyroid.

Total T and DHT are definitely very low. Getting it to 800+ will make a massive difference for you. Also, TSH is too high, which indicates that T3 is mostly too low.
Even if everything else were perfect, you'd likely still have low libido if T and DHT were low, unless you were super sensitive to T, which is highly unlikely.

Here's how to fix it.
#1 Eat at maintenance or slightly above to provide fuel for testosterone production.
#2 Eat nutrient-dense foods to provide cofactors for steroidogenesis
#3 Get sunlight
#4 Optimize sleep
#5 Eliminate stress as best possible
#6 Optimize gut health - gut issues are a big reason for low T, high cortisol and no libido

Supplements can help slightly, but if you don't fix the root issue, it will never resolve. Resolution is the best since then you don't even need supps.

Your Estrogen is very low., many speak about that we need normal estrogen for healthy libido.

Estrogen need not be worried about when one's T is extremely low. Estrogen comes from T. When T is optimized, E will follow.

Something is definitely causing inflammation, which is also stimulating the aromatase. That's a very suboptimal T to E ratio. Maybe things can cause inflammation, but a few common ones are gut issues, sleep apnea and nutritional deficiencies.

You were fasted when you took this test right?
