I think that if you are in top metabolic health, you reject hierarchies and the need to climb them / judge other people by them, because that's a sign of rigid thinking. Psychopaths are the opposite of that - they are not healthy, don't really feel good, so in their mind the only way to feel better is to climb the (arbitrary) hierarchy at any cost. Even if they manage that, there is no happiness there.
