What works for premature ejaculation?

Men, or women who have had partners with PE, what in your experience helped? Or conversely, women with partners with delayed ejaculation, or men with delayed ejaculation, what are some characteristics/habits in your experience that people with delayed ejaculation have that others don't?

Looking more for systemic solutions, as opposed to in-the-moment techniques.

Avoiding porn has been a frequent suggestion in the past on here, but I was hoping to get other suggestions as well.

Thanks!

Back in college, alcohol was an easy quick fix for PE. It was easy to overdo unfortunately (all of a sudden you have the opposite problem) and the long-term effects are deleterious obviously.

These days, only grounding will do. I was back on Dr. Lawrence Wilson's page last week reading through his concept of "down" -- which is simply moving energy down and out of the body, a discharge of excess upward traveling energy, supposedly resulting in energetic harmony. I don't wholly endorse Dr. Wilson's work but there is value in the idea of "downward development" and it ties nicely into the Reichian tradition, including Lowen and Michel and some others I'm forgetting.

I think for PE you also need to practice cycling stimulation and recovery of the penis without ejaculating. Porn wouldn't be out of the question since it is guaranteed to stimulate.

In theory, yes, but the refractory period affects some more than others. Going again too soon can be tedious and/or painful!

A strong sense of self awareness can helpful. Someone who meditates, does some form of internal based type training like Tai Chi, systema martial arts, etc typically aren't as tense. Being more present is much easier when you're not reacting to every sensation and responding by having less tension through your "system."

Plenty other ways, but the ones above are pretty consistent with what I've seen work for myself and others.

Probably lowering cortisol and keeping your dopamine levels in check.

Norepinephrine is high, causing "premature" (quicker) ejaculation. Anything that increases sympathetic activation or norepinephrine could lead to this problem or exacerbate it.

GABA agonists can be very helpful. Sympathetic nervous system stimulates ejaculation, parasympathetic inhibit it. Some people have reported that when they tried my taurine + GABA mix, they struggled to orgasm.
Also, getting your mind away from the excitement of the event (during the evening) for about 2min can be very helpful and make you last much longer when you bring your focus back.
Oxytocin promotes sensitivity and ejaculation and certain supplements with zinc, Rhodiola, folate and B12 have been shown to help with PE.

I have also thought the exact same thing. Like just wait about 10 mins or so, and just go again. I think some men have very delayed reload time.

I did a video on this topic too:

The really quality sex, the stuff that flies over average consumers head, the chemistry..it's all call and response. Best to focus on breathing and going by your senses, some times call for more intense sessions and some call for the dragged out lovey dovey stuff. Sex is an intimate thing, so in a relationship scenario everything is brought to the table, or should be. Going for round 2, changing focus from penetration to eye gazing, for instance is good practice. I don't like the idea of using supplements in regards to sexual performance, atleast not until the party has grasped enough of it and is plenty relaxed and cool about it.

Stress hormones are often involved, and as such anti-stress drugs like clonidine or anti-prolactin chemicals like bromocriptine may help.

I think it depends on the case.

If they avoid porn and ejaculation, and still have this problem...oddly enough exposure to some porn and maybe some edging can help

If they are deep into porn and ejaculation, then getting away from those will help.

Anything that raises dopamine will also help to a certain point. Funnily enough, lots of people who use cocaine have trouble getting it up.

This study you posted referred to low prolactin levels, not high, as related to PE-

"Thyroid-stimulating hormone, luteinizing hormone, and prolactin levels were significantly lower in men with premature ejaculation according to premature ejaculation diagnostic tool (p=0.017, 0.007 and 0.007, respectively). Luteinizing hormone level (OR, 1.293; p=0.014) was found to be an independent risk factor for premature ejaculation."

There's a temporal aspect of PE that's missing from this study. Prolactin rises in the refractory period. The study took a hormonal screenshot of PE men but when was the last time they ejaculated? What are their hormonal profiles in the time immediately following that? Seems relevant. No mention of recent sexual history of the participants, only an exclusion of those with "a sexual intercourse frequency less than once a week."

~

One of the citations in the study posted showed FSH was the risk factor, not LH:

"Mohseni et al. found no correlation between LH level and PE, even though men with PE had significantly higher levels of FSH (9). However, in our study there was no correlation between PE and FSH level, but men with PE had lower levels of LH than control group."

"The serum levels of FT and FSH were significantly higher in cases (P = 0.036 and 0.003, respectively). There was no significant difference between TT, LH and PRL levels of the two groups."

In addition to what has already been said, I would add the following:

Frequent ejaculation is a common cause, because it causes lactic acid and free radicals build up in the pelvic area. If you do it too often these byproducts will not be cleared fast enough and will cause inflammation and muscle fatigue and dysfunction. In young men, frequent ejaculation is associated with high risk of prostatitis.
Once a week is probably a good balance. If you go longer than 2 weeks, you might get PE again from the prostatic fluid build up this time.

Shallow breathing is another cause. Complete exhalation is very helpful because it causes muscle relaxation. Exhaling is linked to the parasympathetic nervous system.

Short frenulum is another possible cause. Some people improve after frenuloplasty.

The Chinese topical ointments seem to be very effective (Suifan Chinese Brush, Pau Yuen Tong Old Chinese Balm). They are typically applied only on the frenulum in tiny amounts and they can delay ejaculation sometimes for hours. The nerve endings in the frenulum seem to be highly involved in triggering the ejaculation process, and it would explain why many people get improvement after frenuloplasty.
