TLDR @ BOTTOM
For the past couple months I had been dealing with a lot of constant inescapable stress and trying to follow almost no-fat diet. I was living off of powdered nonfat milk, apple juice, sugar, coffee, cokes, occasional liver, potatoes, and not much else. As time progressed I began to have constantly sweaty palms and feet and got a strange taste in my mouth when I ate carbs. It also seemed like my glycogen storage capacity was decreasing because I seemed to constantly eat(drink apple juice) to feel ok. I thought maybe I had a zinc deficiency so I would occasionally take some zinc gluconate when the symptoms arose. This seemed to help, but only temporarily, and the symptoms would soon return.
About two weeks ago, concerned that I was not getting enough calories, I foolishly mixed 300 grams of organic cane sugar into my milk and gelatin and drank it. About two hours later I started to feel very ill and light-headed. I sat down on the recliner and started to feel like I was going to black out. I was cold and clammy and my heart was going a hundred miles a minute. I persisted in this state for about three hours, intermittently breathing into a paper bag. I downed three or four tablespoons of coconut oil because I thought it would work as fuel if carbs wouldn't. After three hours the tremors had stopped and I felt a bit better. I had the urge to use the bathroom and I ended up eliminate three very large stools over the course of the next couple hours. A taste of lactic acid was in my mouth and my legs burned like I had just run a marathon.
When I tried to drink some carbs again (apple juice or coke I don't remember) I experienced the same lactic acid burn and some vomiting. I decided to try goat milk and cokes with coconut water and mineral water. I still experienced the lactic acid burn and extreme restless legs along with vomiting. The only relief to these symptoms was raw carrot, but then even the raw carrot stopped working and stopped showing up undigested in the stool. I started to experience constant constipation which I could only relieve with some cascara and a saline enema. The symptoms continued and I got very little sleep (maybe an hour a day). This was the first 3 days or so.
I assumed I had become diabetic due to my lack of carbohydrate cofactors and decided to try and correct it with what I had seen Peat say in his interviews. I started taking thiamine and niacinamide and aspirin. They would alleviate symptoms for an hour or so but nothing stuck. At this point I was still drinking cokes, goat milk, mineral water, coconut water and eating coconut oil as my only nutrition. I decided to try a carbonic anhydrase inhibitor so I started taking some acetazolamide I had on hand. The first time I did this I was able to sleep and woke up thinking I was better. This was day 5 or so.
It did not last, as the days progressed I continued to take acetazolamide, bag breathe and take thiamine, niacinaminde, glycine and aspirin. I continued to have the lactic acid symptoms and endotoxic symptoms (low back pain, restless legs etc). Constipation alternated with diarrhea that smelled acidic. I continued to only get an hour of sleep a day, waking up suddenly in a cold sweat. Since raw carrots did not work anymore, I thought mushrooms and bamboo shoots would work. Sometimes they did, sometimes not, so I stopped all the fibers and just took cascara and enemas. Vitamin K2 and D seemed to offer some relief, but again, it was short lived. Oysters seemed to help as well but not for long. I continued to have smaller shaking/blacking out/cold sweat episodes but less severe than the first one.
I decided that maybe the milk lactose was somehow being turned into lactic acid so I stopped the goat milk and only drank cokes and mineral/coconut water ( I know ) and ate coconut oil by the spoonful. I still experienced some symptoms but they greatly reduced. Any time I tried meat or anything solid (parmigiano reggiano) I would get strong endotoxin symptoms and parts of my food would pass through in my stools (if I could actually go). I continued to have smaller shaking/blacking out/cold sweat episodes but less severe than the first one. This was 9-10 days in.
I added in honey to my foods because I thought it would help store liver glycogen. I started to develop spider veins where my muscles ached with lactic acid burn. My hemorrhoids seemed to have grown distended and my sphincter seemed to have a decline in function. I decided the cascara was contaminated or not even cascara because of the last symptom (hemorrhoids). I got some Pau D'arco and that seemed to actually work. I decided to try drinking OJ and eating oysters very regularly to see if I had acquired deficiencies from the apple juice/nonfat powdered milk diet. The OJ seemed to not give me the lactic acid symptoms but I think it was too acidic as I experienced calcium and magnesium deficiency symptoms. I started making leaf broth and drinking coffee to remedy this but both leaf broth and coffee seemed to give me endotoxin responses. I stopped eating coconut oil and about two days after I had a another almost blackout/tremors episode. When I ate some honey to restore blood sugar, I experienced more lactic acid symptoms. This was about a week and a half in.
I started eating coconut oil again because it seemed like I had less symptoms when I did. I also switched out the orange juice for clear pulp free apple juice. I also started 1 mg of methylene blue a day. This seemed to help but I couldn't eat any protein without endotoxin symptoms. I finally decided to go to the emergency room. They ran my blood work and it was mostly normal. My CO2 was high and one of my liver values was slightly over the high end of range but the ER doc didn't seem concerned and told me to stop taking aspirin (he ran a salyicates test).
So more apple juice and stopped the coconut oil again (concerned about alfatoxins with my high liver value). Tried to eat some cottage cheese. More endotoxin symptoms. Stringy feces. Tried casein powder, more endotoxin. I decided to try some tetracycline (50mg) and I felt the best I had felt so far. The day of the next dose I didn't take it because I was concered my blood levels of calcium and magnesium were too low from starvation and malabsorption...I didn't want to give myself a heart attack.
Finally I started getting lactic acid symptoms from the apple juice and since I was paranoid of alfatoxins in the coconut oil I didn't take some even though I knew it would help. After thinking about the patterns of the lat week and a half I realized I might have overgrowth of bacteria into my small intestine. I finally decided I would have to use a finely grated carrot as an emergency antibiotic so that I could then drink more apple juice or I would faint. I was going to wait two hours after eating the carrot with vinegar and then try more juice but I ended up fasting instead. I booked an appointment with a urgent care doc. I started drinking coconut water and seemed to tolerate the small amount of sugars in it. He would have none of this bacterial overgrowth talk and had me take a H. pylori breath test. After I drank the test solution I seem to not tolerate coconut water anymore.
I became very despondent and decided to try a different emergency room. I had my blood drawn and then was sent back to the lobby to wait. While waiting I had the worst almost blackout/ tremors episode ever. I had to take a wheelchair into the ER room. This one was worse than the first and I was basically accused of being a hypochondriac. They did give me a general referral to a health clinic next to the hospital. I followed up with that the next day and the doc gave me a general referral to a GI specialist. I have an appointment for an endoscopy today. These last 3 or four days I have eaten maybe 4 oz of liver and some beet green broth. I tried some potato soup but even the residual carbs in that gave me burning stools and burning legs. I am down almost 15 pounds since the beginning of this ordeal and my mental function is starting to break down. I am trying to at least keep my alkaline minerals up by drinking mineral water and calcium bicarbonate water.

Depending on if I eat coconut oil I can seem to absorb different lengths of sugars without the lactic acid symptoms. Lactose never.
Protein and soluble fiber consumption gives endotoxin symptoms
Carbonic anhydrase inhibitor gives only temporary relief from lactic acid symptoms
There is undigested food in my stools
I had constipation now I have diarrhea
Vitamin K2 and glycine give only temporary relief from endotoxin symptoms
Even the sugars in scallops and coconut water seem to create a lactic acid response
I now have a dry cough and pressure behind my eyes, also occasional erratic heart beats
I am down 15 pounds
I can only sleep when I fast
My CK level from my last ER visit was high
This has been going on for more than two weeks
I have an endoscopy appointment today and I have been to the ER two times.
I'm thinking this all seems like D-lactic acidosis and bouts of endotoxic shock brought on by lactobacilli and others overgrowing my small intestine. Brought on by poor nutrition, stress, and sleeping in a hammock with my feet higher than my head. Coconut oil can seem to keep a portion of my small intestine clear but it does not eliminate the endotoxin symptoms. I've attached a paper on D-lactic acidosis.
I'm concerned that I will get to a point where I will be so nutrient and calorie deprived that I won't be able to recover my small intestine even after a course of antibiotics. Again I'm getting an endoscopy today but I have no idea how long it will take to get a doc to test for Small Intestinal Bacterial Overgrowth and also understand the d-lactic acidosis issue.
What should I do? I have some tetracycline on hand and I could order some erythromycin but I don't know if I'm too nutrient and calorie deprived to recover the small intestines natural defenses at this point without a medical administered elemental diet. I also probably have low stomach acid.
I'd love to just hand myself over to treatment of a doctor but that is proving harder to do than I thought. We will see if I can talk to someone willing to test for SIBO at my endoscopy today.
