What could be causing these peculiar symptoms?

potatoes/vegetables alone: no issues

butter / fatty meat alone: no issues

potatoes/vegetables and butter/fatty meat together: sweet smelling urine, thyroid feels full and heavy for 60 min

Your thyroid feels full and heavy? Can't say I've ever heard anyone say that before

Yes, kind of like someone pushed your throat inward at the thyroid. It doesn't restrict breathing or swallowing, just feels uncomfortable.

Fat combined with carbs is the primary culprit in type II diabetes. Sugar in the urine is sign of diabetes, although a sweet smell may or may not mean anything. Get your thyroid and blood sugar tested. You can also buy a decent economical blood glucose monitor at Walmart and test blood sugar yourself. But I'd do both just to be on the safe side.

My thyroids were within reference range and blood sugar in the upper end of the reference range last time they were tested. Diabetes is what I have been thinking of too, but these symptoms only appear for 1-2 hours of combining potato & fat, causing the sweet smell only for the first post-meal urination. At any other time neither of the symptoms is present.

It's also interesting that white rice + fat doesn't cause such symptoms. Could it be that the glucose in potatoes is somehow less available?

Of note, there is little evidence that total carbohydrate intake is associated with the development of type 2 diabetes (30,70,73,74). Rather, a stronger association has been observed between total fat and saturated fat intake and type 2 diabetes (75,76), although not all findings are in agreement (30). Additionally, two prospective cohort studies have shown no risk of diabetes from consuming increased amounts of sugar (74,77), and in one study, a negative association was observed between sucrose intake and diabetes risk (72). Intakes of both whole grains (72,78) and dietary fiber (in particular, cereal fiber) are associated with lower risk of type 2 diabetes (30,7072).

And where exactely is the evidence that saturated fat and carbohydrates are "the main culprit"? Neither carbohydrates nor saturated fat cause diabetes in normal amounts, alone or in combination. Observational studies with their questionaires and definition of "saturated" fat are a joke. Polyunsaturated fats, stress, and endotoxin are the main factors contributing to diabetes.

Fifteen patients with Type 2 diabetes were given two diets rich in either saturated fat or polyunsaturated fat in alternate order over two consecutive 3-week periods on a metabolic ward. Both diets contained the same amount of fat, protein, carbohydrates, dietary fibre, and cholesterol. The proportions of saturated, monounsaturated and polyunsaturated fatty acids in the saturated fat diet were 16, 10, and 5%-energy and in the polyunsaturated fat diet (PUFA) 9, 10, and 12%-energy. The PUFA diet contained a high proportion of n-3 fatty acids. Metabolic control improved significantly in both dietary periods, due to both qualitative dietary changes and a negative energy balance. The serum lipoprotein concentrations decreased on both diets but the serum lipids were significantly lower after the PUFA diet (serum triglycerides -20%, p = 0.001; serum cholesterol -5%, p = 0.03; VLDL-triglycerides -29%, p less than 0.001; and VLDL-cholesterol -31%, p = 0.001) than after the saturated fat diet. Average blood glucose concentrations during the third week were significantly higher fasting (+15%, p less than 0.01), and during the day at 1100 h (+18%, p less than 0.001) and 1500 h (+17%, p = 0.002) on PUFA than on the saturated fat diet. Significantly higher blood glucose levels were also recorded with a standard breakfast, while the sum of the insulin values was lower (-19%, p = 0.01). It's not as simple as carbs + fat causes diabetes.

It should be rephrased as such - in someone who lacks metabolic flexibility (The ability to use both carbs and fats as fuel simultaneously), carbs+fats together can be dangerous, especially if the fats are PUFA's.

Most people who are metabolically flexible can switch between fats and carbs just fine, as long as the fats aren't heavy in PUFA. I don't think metabolic flexibility can be easily restored, once compromised, until PUFA depletion occurs.

However, many people are not healthy, not metabolically flexible, which is why the frequent recommendation is to go low-ish fat and high carb to avoid the possible damaging effects of the so called "randle cycle" while carb sensitivity is restored and body weight is normalized / PUFA is depleted & metabolism is fixed.

by eating carb/fat/meat together you are increasing your insulin response. making it go higher and for longer. this shuts down methylation/adrenals which shuts down thyroid. you may also be increasing insulin resistance and that means your blood sugar goes high and that creates the fruity taste in your mouth. this has been a marker for diabetes.

The reference range for TSH tends to be wide, and for some people too wide. Signs and symptoms count too.
If you want, you can post the lab test result numbers.
You can read some of Peat's articles on thyroid on his site raypeat.com.
And you can measure some things yourself - for instance your body temps, your resting heartrate, and a rough daily maintenance calorie average. These can give some more clues about metabolism and thyroid function.
