A friend asked her doctor about taking Progest-E or black cohosh for menopause symptoms. He told her no way since she smoked she couldn't take any hormonal stuff as it put her at risk for blood clots.
I think this applies to artificial hormones.
    Any advice?

Progest-E would help clots, not worsen them. RP's recommendations in general including progesterone, vitamin E, aspirin, thyroid, and caffeine, would help your friend if she has a problem with blood clots.
