I am posting this purely out of scientific interest. Not interested in joining the abortion debate in any way. Apparently, when progesterone is given in higher doses soon after the abortion pill, many women will not proceed to abort the fetus and will give birth to a healthy offspring. The progesterone dose used to reverse the effects of the abortion pill RU486 (mifepristone) was 200mg, given as an IM injection.

Apparently abortions only happen in 60% orf women who miss the second dose of meds (like the 6 in this study) anyway.

I thought RU486 was similar structurally to progesterone?

It is, it just has a strange bulky molecule attached at position 11 on the steroid ring and this is what makes it anti-cortisol. In general, oxygen as position 11 will make the steroid anti-cortisol. So, 11-keto DHT and 11-keto T would be two examples of steroids that inhibit 11b-HSD1 and thus the synthesis of cortisol. Other steroids can inhibit cortisol as well but probably indirectly, while 11-keto steroids are usually direct 11b-HSD1 inhibitors.

Ray has written about ru486 as an anti-progesterone.

Some of its effects are anti-progesterone but its much more important clinical effects are glucocorticoid antagonist and estrogen antagonist. This is why it is showing promise for treating some cancers and of course Cushing disease. If you search Ray's site for RU486 you will find the part where he described that it is not really a strictly anti-progestin drug.

That is what I thought...that there were some positive comments on it from Ray. Thanks for the details Haidut. Amazing that the detail you bring is getting to the "oxygen at this junction brings down cortisol" point.

This is consistent with DIY chemical abortion using frequently dosed ascorbic acid (every 60 to 120 minutes around the clock, 24 grams/24h) to pause the very high progesterone secretion post implantation so that the fertilized egg is aborted. From personal experience it really works, but the window of effectiveness is very short.
