About 2 years ago an Australian study hit frontpage news. It found that ingesting 1g niacinamide daily significantly reduced the risk of non-melanoma skin cancers.
Vitamin B3 Prevents And Reverses Skin Cancer

Now, a new study and a review found that the same dose of niacinamide may work for melanoma as well, and in facts recommends preventive administration of this dose of niacinamide in high-risk individuals.

Melanoma and Nonmelanoma Skin Cancer Chemoprevention: A Role for Nicotinamide?
Vitamin therapy could help prevent skin cancer - The Statesman

"...Niacinamide (nicotinamide) is a form of vitamin B3 (niacin) and is used to prevent and treat niacin deficiency (pellagra). Niacin deficiency can cause diarrhoea, dementia, tongue redness/swelling, and peeling red skin. The findings showed that nicotinamide can help reduce or reverse DNA damage, inflammation, and immunosuppression caused by ultraviolet radiation. "Nicotinamide has been shown in a clinical trial — called ONTRAC — to reduce the incidence of non-melanoma skin cancer in high-risk individuals," said Gary Halliday from the University of Sydney in Australia.

"...Ultraviolet radiation causes DNA damage in melanocytes — melanin-forming cell, especially in the skin, and leads to the production of reactive oxygen species. These species induce inflammatory cytokines that, together with the inherent immunosuppressive properties of the ultraviolet radiation, propagates the initiation of cancer formation. "As nicotinamide reduces the incidence of actinic keratoses and nonmelanoma skin cancers in high-risk individuals and enhances repair of DNA damage in melanocytes, it is a promising agent for the chemoprevention of melanoma in high-risk populations," Halliday said."

I have found that this is absolutely true: I began this regime after the last time the dermatologist used liquid nitrogen to remove AK and unlike previous times, they have not returned while taking Niacinamide.

Melanoma and Nonmelanoma Skin Cancer Chemoprevention: A Role for Nicotinamide?...
At first glance, Diona Damian’s August 2017 review of niacinamide and skin cancer prevention seems quite good. Her group has researched skin benefits of niacinamide for at least several years.

Also see the recent Parodi group and other links below.

Nicotinamide for skin cancer chemoprevention. - PubMed - NCBI
“…patients receiving nicotinamide showed a 23% relative rate reduction in new nonmelanoma skin cancers (BCC [basal cell carcinoma] and SCC[squamous cell carcinoma]) compared to placebo (P = 0.02); the unadjusted relative rate reduction was 27% (P = 0.02). There were similar magnitudes of reduction for both BCC (relative rate reduction 20%, P = 0.12) and SCC (relative rate reduction 30%, P = 0.05).

“Nicotinamide at pharmacological doses has a long history of clinical use. It is inexpensive, widely available commercially and has an excellent and well-documented safety profile. Phase 3 evidence of its chemopreventive efficacy against nonmelanoma skin cancers in high-risk patients means that it is now becoming part of the routine management of this group.”

Prevention of non-melanoma skin cancers with nicotinamide in transplant recipients: a case-control study. - PubMed - NCBI
“Among these 18 patients, seven patients (42%) had shown complete clinical regression and no patient developed new AKs [actinic keratosis]. Conversely, among the controls, 91% showed an increase in AK size and/or developed new AKs.”

“…NCT[niacinamide], at all tested concentrations, strongly down-regulated COX-2 mRNA and protein levels 24h after irradiation with UVB 10 mJ/cm2. Irradiation with UVB 60 mJ/cm2 resulted in a lower COX-2 increase, nevertheless NCT was able to reduce its expression.”

“NAM[niacinamide] rescues glycolysis but not OXPHOS. In cultured cells, simulating oxidative stress by a short H2O2 treatment leads to massive differentiation after 5 days, which can be efficiently prevented by NAM. Additionally, we found that NAM partially prevents HPK senescence induced by oxidative stress conditions in vitro.”

Thanks.
Interestingly, the company in the USA that pushed nicotinamide riboside (NR) has done another trial with various doses of NR and has determined that up to 1,000mg daily is optimal for humans. I don't know if they are doing this on purposes to match the skin cancer trials which also used 1,000mg or of there is indeed an optimal dose for most conditions. When Peat was asked about niacinamide safety he said up to 1,000mg daily would be safe for long term consumption. So, a few diverse sources all converging on the same range...

I have lots of AK on scalp, face, forehead, hands, back and kept it successfully at bay for years by applying 2% iodine once a month....after now keeping my niacinamide level at min. 100mg daily, it has greatly diminished...can take my daily tropical sunbathe without problems.
