I just want to warn you that high doses of niacinamide are going to shut down your methylation and deplete methyl donors.
This will prevent breakdown of adrenaline, estrogen and histamine and also lead to endothelial dysfunction.
If your estrogen builds up, your RA will feel better because estrogen = ceruloplasmin and ceruloplasmin protects from arthritis, but estrogen slows your metabolism.
Body can only afford to raise estrogen (ceruloplasmin) without negative consequences when your metabolism is high.

Oh my! This is certainly something new:( Since I am quite new at this don't really have enough understanding for the biochemistry but understood from Haiduts earlier post on RA being a metabolic disease with a dysfunctional glucose metabolism that in fact was reversed in rats with high doses of niacinamide? No mention of effect on methylation. Have to go back and see....Thank you for the input.

Good luck. I hope you'll find your way. It is a pity that doctors are useless. They only give precriptions without even thinking about other options to try.

Yes you do feel quite alone with it all...:(

I wish somebody would just make all this food for me that I need! It's such a chore...
Me too. :)
Once you figure out some things that work, and if you have a good fridge and freezer, you can make batches of food and store meals for future eating. Which helps, but it's still work. I found it a lot easier when I found I could use more fruit and juice for snacks. Dried fruit carries and stores well for emergencies.

Did som research and this is what I found:

Investigators found a very high correlation between breast cancer recurrence and the amount of methylation of a gene called PITX2. In breast cancer patients with low methylation rates, 90% were free of cancer for ten years, while patients wirh high methylation rates only 65% remained cancer free for that length of time. Niacin provides one of the few natural powerful methyl acceptors in the body".
So low methylation is not so bad? Not in cancer anyways? Which would be strange as I have understood that excess estrogen is usually a powerful culprit in breast cancer?

Thanks for your reply. Have run accross it in my desperate search for answers and understanding. I have tried targeting different possible causes. Think my metabolic rate has improved but otherwise still fumbling in the dark. Next project is to see what bloods say and meating with a thyroid specialist ( Bit nervous for this encounter) since need to have verification that I would benefit from T3. I will however put the results to inspection on the forum before meeting this specialist.

Get so very confusedo_O If you have low metabolic rate and dysfunctional glucose metabolism and cell energy production, I can only understand that fixing this is the first step and OJ I believe at least is quite central as is aspirin and progesterone and coffee but then the dairy for someone with RA is for me as well a question mark since the protein in dairy is quite allergenic and really not tolerated by many with RA it seems. So now I'm leaving out dairy compleately for a month to see if difference in pain and stiffness. I have been taking high doses of niacinamide for a while but have now changed to niacin instead after having read Abram Hoffners book: Niacin, the real story. I find that in fact the flushing effect really brings heat to the surface which is also seen as important in RA in Chineese medicin. Niacin does not make me nauseous eather like niacinamide. Niacin in fact has made a big difference already in my head. It feels so much clearer and there is a noticable difference in memory capacity.

Raising the metabolic rate, the digestive fire (agni), is central to Ayurveda. But AFAIK, they don't recommend ingesting large quantities of orange juice for this purpose.

Try to be more natural and eat whole fruit instead of juice. It'll keep your intake to a reasonable amount. Large amounts of sugar will create dampness, according to TCM.

I noticed the improvement in mindfulness and focus from Niacinamide too.

I still drink milk.
I think it's easy to get lost in the weeds but it might be easier to implement if you keep things simple by just tracking your temp and pulse according to your diet.
And maybe instead of guessing at all the possible triggers, do a very simple elimination diet; like maybe just potatoes, OJ, gelatin broth, eggs, sugar, fruit and seafood. Very basic, eat the same handful of things for a couple weeks (with minimal, or no supplements) and then add in one food element at a time.

Do you fill out a food and symptoms and supplement log daily? That makes it a lot easier.

Thank you for your reply. Actually this is what I am doing pretty much. I am keeping it simple and have done it for such a long time in different ways. The leaving out of all dairy is new, so is the niacin. I really feel quite a change now and will go on like this for a while. I need to take aspirin still 3x500mg a day for pain. No longer need NSAIDs, think that's going in the right direction(?) Measuring the MR gets a bit difficult though when taking aspirin since it does affect temp.

Anybody suffering from RA owes it to themselves to try thyroid. My personal suggestion is @haidut 's tyromix.
