Sinus tachycardia is a normal rhythm with a high heart rate. A prolonged QT is when the amount of time it takes the heart ventricles to contract and relax is longer than normal. Not sure on the T wave but here’s an image that might help.

I’ve read here that a prolonged QT can be due to hypothyroidism. There’s other things that can cause it as well though like electrolyte imbalance and medications just to name a few.
