Hypothyroid or not , any ideas?

Fatigue issues since 2010, very ill in 2015. Doctors palmed off as CFS and sent me to a pain clinic.
Fast forward 6 years and i've followed various protocols and diet, got rid of a lot of my unbound iron, increased copper and other nutrients. Digestion now so much better and energy 1000 times better.
Tried numerous doctors, neuro, endo, rheumatologists, acupuncturists, bowen therapy, chiropractors, muscle testers (suggested removal of my one of my 10 amalagams which i have booked to be done), NAET, homeopathy and various alternative medicine specialists and various other things to no avail on the below:


Issues i'd like to be resolved:
Leg exercise intolerance (hit lactic threshold early) from /incline walking/stairs,cycling, squats etc
Haven't slept through the night for 20 years. Wake up around 3-4 times on average. Tried anti histamines, blue light filter glasses and various snacks before bed
Carpal tunnel type symptoms started 15 months - pain in tips of right hand first 2 fingers and thumb, typing/phone use very painful.

Decided I'd try the GP again in regards to my carpal tunnel as it's becoming hard to type and therefore work, see if I can get a referral for nerve conductivity tests/ bloods etc

Always speculated I may be hypothyroid due to the fatigue and similar symptoms. These are my bloods which are quite old, other than asking for thyroid bloods again - TSH/reverse t3/t4 is it worth checking on anything else?

Is it worth just trying a natural throid supplement and just seeing what happens ?

Yes. My TSH was about the same as yours when a doctor knowledgable about the thyroid told me I was VERY hypothyroid. Sometimes the TSH is not sky high because stress hormones keep it down. I think that's what happened to me.

Your temperatures are low too.

Are you able to do the ankle reflex test with someone to help you? There are videos on YouTube that show you how to do it and give you an idea what a normal v. a slow reflex looks like. It could be helpful, but I think you gathered enough info to try a thyroid supplement. Just try it slow, the Broda Barnes way, and with magnesium. Your muscles can cramp up without sufficient magnesium, and I think this can apply to your heart too from what I remember Dr. Peat saying on one of the shows.

not sure if I'm missing it but what is your vit d ? b12 ? and i would totally add some iodine. 10 amalgams is a major issue - but im sure you know that. get those things out asap.

Why iodine? Dr. Peat said it's pretty much never needed and best not to take it. It can harm your thyroid.

Yes. I understand and to a point disagree with him on that point mainly bc it really has helped my thyroid more than any meds ever. In fact , all I take now to keep my thyroid super healthy ( temps 98.6 - great heart rate , etc ) is coffee with milk and sugar ( a ton of it ) and iodine , B12 sublingual , and Vit D and C and dairy. I dont get near enough iodine from my food so I supplement with nascent iodine 3 drops a day. I do eat foods with selenium . Iodine is the single more important thing for me , it might not be for you. I was raised on the beaches of Southern Cal , have Northern Euro blood , and right now I am nowhere near the water. When I stay in NYC , I tend to not need as much. Iodine detox is gnarly with a bunch of metal in your mouth . Those amalgams need to go .

Both thyroid and thiamine are required for healthy oxidative metabolism so a lot of the symptoms overlap. But the lactic acid issue and the nerve inflammation issue scream thiamine deficiency to me.
Tried high dose thiamine off the back of research by Mars and Lonsdale. I started titrating allithiamine over an 8 month period up to 1000mg and 1000mg HCL for several months and saw no benefit. I titrated back down and am on around 100mg of allithiamine per day, I don't see any benefit going over that and there is more risk causing a knock on effect to other issues. I found after increasing thiamine I did feel slightly better again, like another gear change, so stuck with it.

What supplement are you using ? I presume you start with half a grain ?

When I got up to 2 grams/day, major improvement happened within a couple of days, like a switch had been flipped on my autonomic nervous system. It is my understanding (from Elliot Overton and also from Dr. Chandler Marrs) that TTFD requires glutathione to work and uses it up if you are low to begin with which is problematic. I think that was my problem with TTFD. Thiamine hcl is recommended if glutathione levels are low.

I read yesterday that high dose thiamine hcl does cross the blood brain barrier if the dose is large enough to flood the body (via perfusion?). Of course, now I cannot find that article. If I find it, I'll add it to this post.

I've also learned that thiamine deficiency causes the blood brain barrier (bbb) to become compromised: Error - Cookies Turned Off If the bbb becomes leaky, heavy metals like iron get in and wreak havoc. Healing the bbb is accomplished by supplementing with thiamine.

I forgot to mention I did stop allithiamine for a month and focused purely on HCL. I got up to 1000mg of sulbutiamine and started taking 0.5-> 1 gram of HCL powder as I read some studies that were only performed on HCL and wanted to expirement. i experimented for about 3 weeks as i slowly titrated. I didn't really notice much difference I was only testing it as I was hoping it was prevent my premature lactic acid burn in my thighs that i was experiencing and it didn't.

Interested to know what this reversed for you? I was actually rotating allithiamine / hcl 2 weeks on and off, but i just realised for the past 6 months i have just been taking allithiamine. should probably get back on the hcl cycle

I didn't go above 1000mg as I noticed no difference and by that time with the previous thiamine supplementation (8-12 months) and then adding mimosa pudica for 3-4 months I cured my chronic loose stools, which is what I wanted.
When you state lactic acidosis what symptoms did you have? Did you experience the same issue as me? - Lactic acid burn starts occurring when performing say 10+ unweighted squats.
Throughout all the thiamine supplementation I didn't experience any relief for the carpal tunnel, if anything I remember it actually being a bit worse and I think why I lowered my supplementation down as I read too high a dose can then deplete B2 etc and I was feeling so good I didn't want to taint anything.

I was unable to function. the lactic acid was very bad, no attempt at exercise was necessary; it was constant and unrelenting. It hurt to try to roll over in bed. I could barely walk across the room.

What you are describing is how I was for decades. Then I got more damaged via Bactrim antibiotic in 2020 which blocked my thiamine function further and almost killed me.

It was my understanding that carpal tunnel is supposed to be caused by a B-6 deficiency. I've read that thiamine deficiency interferes with b6 somehow. Maybe try taking a good b-complex? The b vitamins seem to need each other to work well. In addition to the 2 grams of thiamine hcl, I also take a b-complex, some magnesium and I drink orange juice for the potassium as these things work with the thiamine.

I take pure synergy - super b complex as well as the bee pollen/ nutritional yeast and liver. - 250mg malate (jigsaw srt) per day.

I started with this level of lactic acid that i have now about 10 years ago, it slowly got worse till 2014 where i struggled to static stand for more than 30 seconds. I went Keto and followed morley robbin rcp; reduced unbound iron, clean diet, magnesium etc. within 2-3 years i was so much better. now it's really only affecting my exercise performance like it did 10 years ago.

How long did it take for your lactic acid symptoms to go away at 2grams ? I am willing to titrate back up to that on purely hcl (not benfothiamine or allithiamine) as an experiment again if you found it beneficial.

I took P5p (50mg) + the 25mg in my complex daily for 100 days (how long the bottle lasted, which finished about 2 weeks ago) and notice no difference. I would have thought I would have noticed something if it was working for me. - I read you have to be careful your body doesn't have too much as you could be inadvertedly causing the issue by already being toxic. I stopped my b complex for 3 months to test that too and not much change.

My lactic acid problem disappeared at 300mg, within 45 minutes, and my temperature went up a degree to 98.6 too. But it stopped working after a few days. So I increased the dose which worked for a few more days, then stopped working; so I increased the dose again. I stair stepped up like this for about 3 months. By January I was up to 1500mg/day and getting concerned I was going to hurt myself with such a high dose. Then I found Dr. Costantini's site and went ahead and increased to 2 grams/day. I tried taking 2.5 grams for one day but I got shooting electrical zapping pains in my thighs so I backed off the dose back to 2 grams.

What do you mean by "reduced unbound iron"? I think Ray Peat would warn you about the P5P; I think he said to keep it low (25mg?); he likes low dose pyridoxine (? I mean plain old B6) better; he said it's safer. I don't know anything about malate; a quick search showed Peat is OK with magnesium malate.

I learned mainly via Dr. Costantini to only take thiamine with plain water and take it away from anything sweet and also avoid coffee and tea for at least an hour because these block thiamine. Just some things to consider. I learned a single cup of coffee made me feel terrible so I gave it up. I cut back on my sweet stuff intake. I still have orange juice though, and a little maple syrup in my milk, and a little honey in my salad dressing. I'm extremely sensitive to nightshade vegetables; they cause inflammation. I avoid high serotonin foods too because of the inflammation. Maybe you have a few food sensitivities that might be causing inflammation?

I was diagnosed with rheumatoid arthritis in 2015 and lost the use of my thumbs. I was really in trouble. But I found Ray Peat and got my thyroid med optimized and recovered pretty much. Now I can walk a mile and then do some strength exercises for maybe 30 minutes afterwards. HOWEVER, I do better exercising if I take a gram of thiamine and then a gram of aspirin before I go walk. Otherwise I have a hard time climbing the hill. Things aren't perfect but considering my age (71) and my history, I'm doing pretty well.

When I stay in NYC , I tend to not need as much. Iodine detox is gnarly with a bunch of metal in your mouth . Those amalgams need to go .
Hey I noticed you say nascent iodine. How do you respond to Lugol’s? When I take Lugol’s internally at doses much greater than RDI I get chills, thyroid discomfort, bad cystic acne. If I apply it topically the problem is less severe, however some of it evaporates before it has the chance to absorb, so that’s a lower dose and it’s hard to control how much you’re getting. I’m thinking of switching it up with nascent iodine.

What supplement are you using ? I presume you start with half a grain ?

Yes, half a grain is a safe starting dose, and a good dose to try, given that it doesn't look like you have severe hypothyroidism from your ankle reflex. Then you can see where you are in 2-4 weeks and see if you want to try more.

Since I seem to have a stubborn case of hypothyroidism, the desiccated thyroid worked only partially for me. I seem to have a major T4 to T3 conversation issue. I have a double genetic mutation on the gene coding for D1, which is responsible for the T4 to T3 conversion at the tissue level, but I don't know how much that accounts for my issues. My mom also has severe thyroid issues, and my dad has some too, and it seems that I inherited their poor hormonal balance, which was difficult to overcome. I now use slow-release T3 every 12 hours, plus a bit of T4 every day. Perhaps now that these thyroid supplements are working for me I could try desiccated thyroid again or Cynoplus, but I don't want to mess with something that is currently working for me.

When I did use desiccated thyroid, I tried Nature-Throid at first. Then I tried all the different Thai brands because they're cheaper, and they worked the same.

Yes. I understand and to a point disagree with him on that point mainly bc it really has helped my thyroid more than any meds ever. In fact , all I take now to keep my thyroid super healthy ( temps 98.6 - great heart rate , etc ) is coffee with milk and sugar ( a ton of it ) and iodine , B12 sublingual , and Vit D and C and dairy. I dont get near enough iodine from my food so I supplement with nascent iodine 3 drops a day. I do eat foods with selenium . Iodine is the single more important thing for me , it might not be for you. I was raised on the beaches of Southern Cal , have Northern Euro blood , and right now I am nowhere near the water. That's interesting. I have only lived in coastal areas since I was a kid, so I probably have gotten plenty of iodine and had no reason to question Peat's advice on this. I have also always eaten a lot of ready-made food, which is made with iodized salt, in the U.S. at least, so even though I don't use iodized salt at home, I do get plenty of iodine.

Before I was introduced to Dr. Peat, I did take an iodine supplement for a few months, but I had no improvement. Probably because I was not iodine deficient to begin with.

My lactic acid problem disappeared at 300mg, within 45 minutes, and my temperature went up a degree to 98.6 too. But it stopped working after a few days. So I increased the dose which worked for a few more days, then stopped working; so I increased the dose again. I stair stepped up like this for about 3 months. By January I was up to 1500mg/day and getting concerned I was going to hurt myself with such a high dose. Then I found Dr. Costantini's site and went ahead and increased to 2 grams/day. I tried taking 2.5 grams for one day but I got shooting electrical zapping pains in my thighs so I backed off the dose back to 2 grams.

What do you mean by "reduced unbound iron"? I think Ray Peat would warn you about the P5P; I think he said to keep it low (25mg?); he likes low dose pyridoxine (? I mean plain old B6) better; he said it's safer. I don't know anything about malate; a quick search showed Peat is OK with magnesium malate.

I learned mainly via Dr. Costantini to only take thiamine with plain water and take it away from anything sweet and also avoid coffee and tea for at least an hour because these block thiamine. Just some things to consider. I learned a single cup of coffee made me feel terrible so I gave it up. I cut back on my sweet stuff intake. I still have orange juice though, and a little maple syrup in my milk, and a little honey in my salad dressing. I'm extremely sensitive to nightshade vegetables; they cause inflammation. I avoid high serotonin foods too because of the inflammation. Maybe you have a few food sensitivities that might be causing inflammation?

I was diagnosed with rheumatoid arthritis in 2015 and lost the use of my thumbs. I was really in trouble. But I found Ray Peat and got my thyroid med optimized and recovered pretty much. Now I can walk a mile and then do some strength exercises for maybe 30 minutes afterwards. HOWEVER, I do better exercising if I take a gram of thiamine and then a gram of aspirin before I go walk. Otherwise I have a hard time climbing the hill. Things aren't perfect but considering my age (71) and my history, I'm doing pretty well.

I had a ferritin of over 374ng ml as you'll see above initially.

I take 25mg of oridinary b6 within my B complex, but I have tried p5p for 3 months and noticed no benefit. therefore it's unlikely i am b6 deficient.

I think you had a clear B1 deficiency issue, especially with lactate build up without even exercising. I am glad you found relief. As mentioned above I titrated up to 1500mg of B1 HCL + 600mg of allithiamine over a period of 8 months and noticed no difference to that lactate issue i have in my thighs or sleep or carpal tunnel. I did initially notice digestion improving and sleep (improve) but i still wake 3-4 times a night.
This conversation has reminded me to mix hcl back in again though
