Mostly the anti-histamine effect increasing appetite and causing drowsiness.
Majorly, as echoed here, it's the anti-histamine effects in the central nervous system; cyproheptadine strongly blocks the H1 receptor in the brain in the same way as diphenhydramine or other OTC sleep-aids.
