@haidut and everyone else

For a young guy (late 20s) who is hospitalized with delirium, with all medical tests coming back as normal (with the exception of orientation to time, place, person), what do you think can be tried to see if it helps?

The cause is not known. He had some minor "brain fog" issues for years leading up to this. Don't know if it's related.

Could be from hypothyroidism that might not be detected by blood work. Low vitamin D can also cause severe mental issues.

Peat once suggested in similar context "an antibiotic such as neomycin to disinfect the bowel, laxatives, enemas, and intravenous glucose would be very helpful".

Small doses of T3 could help (few mcg).

Were heavy metals checked in urine and blood?
Has he had severe emotional trauma before?

Also Niacin (not Niacinamid) would be worth a try as it is known to terminate hallucinations from LSD and some types of schizophrenia. Would start with 2 to 300mg and wait an hour for effects. If none you could try 500 to 1000 mg.

Why do you ask about the severe emotional trauma? Can that cause so serious problems as delirium?

Yea, extensive medical tests, including metals in urine and blood, have been checked.

Why nicotinic acid and not Niacinamide?

Serotonin is now suspected as a major cause of delirium, at least in hospital patients. Cyproheptadine and famotidine have been successfully tried, and thyroid may help as well.

Would thyroid help because it lowers serotonin? I'm assuming you are talking about the active T3 form?

Between Cyproheptadine and famotidine, which would be safer?

T3 could help but the studies were with anti-serotonin drugs. Famotidine is probably safer but has less evidence for treatment of serotonin syndrome. Cypro is the gold standard for that.
