@Matt1951 I have a wife. Yes, I can always go again, but when I have, I've not found the issue particularly improved.

@Richiebogie good advice. I only added Zinc once I started experiencing the problem, before that I didn't take too many supplements. I've probably adopted a somewhat scatter-gun approach, looking to see if anything solves the problem. As you advise, I should be more systematic. I do periodically use cron-o-meter mainly to check calorie intake and macro ratios. I probably don't pay enough attention to the micro nutrients.

@raypeatclips good shout about liver. I did for a while add liver to my diet, but it didn't have a noticeable effect. I suppose then I got lazy with cooking it and stopped eating it. I shall add some back into my diet.

@redlight I agree that the testosterone level, although within range, is on the low side. As I was drinking the night before and had had little sleep for the previous couple of nights (yes, that wasn't too smart!) and the test was at mid-day, it is possible it was suppressed from it's more usual value. I think I will pay for a full hormone test privately. This will give me a more reliable testosterone baseline and, also, provide my estrogen level. I don't claim to fully understand blood work, but other than the lowish testosterone reading, the other results seem ok. Prolactin seems to be at a reasonable level. So I would take that as a good indicator?

@mujuro My job is nowadays almost exclusively office based. So I spend large portions of the day sitting. For exercise I lift weights at home 3 times a week. I also walk. Cardio does not work for me and leaves me with Post exercise malaise. In fact, at one point any exertion would result in this. So Peating has brought me a long way. I'm significantly bigger and stronger, although some of the additional weight is fat.

I first noticed that my penis felt weaker or numb at the start of the year - that has mainly passed. But this coincided with adding squats to my exercise. I have a herniated disc in my back and squats can inflame the nerve pain down my leg. There may be no link here. I removed squatting completely and did weight exercises seated to remove strain from my back and pelvic floor. I've also had the odd 2 week period off working out, and I've noticed no difference.

I've tried all supplements, "niacinamide, aspirin, D, A, K, taurine, caffeine, glycine". I drink a lot of coffee, for a while I was using niacinamide daily, I use taurine and drink Red Bull, I've tried glycine amongst other amino acids, I've been using Haidut's Estroban and I've been a long term advocate of aspirin. However, as I mentioned, I should definitely be more structured with my approach.

I'm not sure I really ever notice much response from supplements. I found my health and outlook improved mainly under the milk, oj, gelatin. I gained weight, increased muscle mass and felt much healthier. I wonder if the tweaking thereafter has actually been counter-productive. What I could do is be more strict and ensure complete removal of PUFAs. I still will eat at restaurants etc. so I'm not in complete control, but this is my compromise to reduce stress.

I suspect it is possible that raised estrogen may be to blame. But I'm not sure why it suddenly became an issue? Resolving the problem is much harder as it potentially could be a number of things: Pelvic Floor and/or nerve impingement; hormone imbalance; anxiety.

People could say and think whatever they want, they can presume premature ejaculation is this, that, these. Nobody really knows, trust me.

I think dopamine is the key to solve this problem.
if you have high dopamine level ,you can fully control yourself.

Sounds like disorder of the hypothalamic-pituitary-adrenal-testicular axis.
high caffeine intake, excessive stress, cortisol and epinephrine -> exhaustion of the dopamine-acetylcholine system. And results in CNS fatigue, and in your case PE.
The serotonin-gaba system cannot modulate dopamine-acetylcholine interaction anymore.
This leaves you with high levels of serotonin and tryptophan metabolites and cause brainfog.

This is something interesting I notice with myself. I drink a lot of fluids (milk, OJ, a can of red bull, quite a lot of coffee) but often feel dehydrated. I also add salt to food, but not in ridiculous volumes. I've tested with increasing salt intake. There have been occasions where I've experienced brain fog (or something similar) and found drinking a significant volume of water seemed to clear it. Now this doesn't tally with the world of Peat.
Acetylcholine disorder can lead to dehydration and impaired cognitive function.
Increase uptake of choline, eat eggs / L-Carnitine or ALCAR and modulate the acetylcholine release with White or Red Ginseng.

Do not train muscles to failure, but keep 3 reps away from it.
Train every 96 hours if you are in your 30-40's, to get super-compensation of muscle growth and prevent tendon injury.

I think you’re onto something; in BBers it’s common to see a mild raise in prolactin which freaks them out - they start taking dopamine agonists and then they report never being able to ejaculate.

I’ve tried stuff like trenbolone and indeed there is full control but my prolactin doubled (albeit still in range). A major confounding factor is that trenbolone is an extremely potent androgen so the full control could be from the lessened estrogen effects. It also vastly decrease serotonin.

So more dopamine less estrogens, which is quite ubiquitous at this point

It sounds weird but my libido,sensitivity and orgasm are not good on Androsterone which supposed to be precursor of DHT.
maybe just my Estrogen got tanked.

btw I had great experience with Vitex+P5P+Mucuna(product name is 'Inhibit P') the combination of prolactin inhibitors.
When I was taking it,I can fully control myself and I didn't really have Refractory period

As an update, I had further blood tests done in the middle of last year. I don't have them to hand, but my testosterone was something like 18 nmol/L 8-29, not great, but a bit more reasonable. I suspect the previous alcohol-fueled late night did crash my testosterone.

What they did pick up was that my TSH was at something like 0.01 mU/L. They were mildly concerned about this and re-tested it for a while. My T3 was ok. Eventually my TSH increased and was above 3 mU/L, which was worse than before. So don't know what was happening there.

Anyway, I tried plenty of things for the ejaculation issue: Lisuride to increase dopamine; other supplements to increase testosterone or dopamine (B6, Taurine etc.); serotonin antagonists like cyproheptadine; pansterone; androsterone; progestene; different amino acids, etc. I altered my exercise; stopped exercising. Altered my diet. Added dates, upped protein, gained weight, lost weight. Nothing seemed to have an effect.

It then eventually seemed to correct itself. So it may have been related to tight pelvic muscles (I did try exercises that didn't seem to have an effect.) Or possibly CNS fatigue. What I did notice was that my testicles were consistently tight to my body. This sometimes returns, and I can't quite tell what causes it.

After it improved, I then suddenly developed an issue where the end of my penis felt numb when erect. There was no problem with erection quality, but the sensation had changed. I returned to the GP (which resulted in the above blood tests among others), who felt I should be checked for MS. I'm awaiting results, but the consultant was not concerned. As an aside, I tried a regime of niacinamide and aspirin, that may have contributed to the numbness. Or maybe not.

I'm not sure how it's functioning atm! Anyway I do wonder if tight muscles and tight scrotum were/ are contributing.

Pelvic nerve problems and penile function, by too low estrogen and acetylcholine activity (needed to keep nerve function) can be helped with ALCAR and forskolin, combined with Damiana, to support pelvic nerve area function recovery.

-ALCAR Increases levels of neurotrophins such as NGF (R, R2) and improves nerve regeneration in rats (R).

-Treatment with the active form of vitamin D for 24 hours increased the NGF concentration in the brain 2- to 3-fold. Pretreatment with Forskolin doubled the results, so that NGF was multiplied by 6 (R). However, this effect could be impaired if your blood sugar is high (R).

-Damiana Leaf (methanolic extract) appears to be a weak phytoestrogen, acting on the estrogen receptor with 9% efficacy at 250mcg/mL; this was attributed to the compounds apigenin (7-O-β-d-glucoside), Z-echinacin and pinocembrin with IC50 values of 10, 20, and 67uM respectively.[7]

So definitely the dopamine being high rather than pure prolactin/androgen/estrogen. High dopamine lowers prolactin and vice versa anyway. Psychologists had it right the whole time “it’s in the brain” lol.

So I think increased penile sensitivity for me was associated with high estrogen, high estrogen induced prolactin, and elevated adrenaline and low GABA and DHT. As when I took p5p and mucuna pruriens it did't do anything for me. I also ate a lot of eggs daily, 12 whole, a few months ago, and it didn't reduce sensitivity. Only thing that helped me was/is andro (I'm still using it).

this is stupid, you can take all the androgens that you want, but if you do not displace the estradiol from the SHBG, that competes binding with DHT, you will not have any improvement and only cause excessive androgens that binds even more SHBG. And then the ratio will drop and get in vicous circle.
So you need to drop loading androgens and damaging NO supplements.
