I would like to ask you for help with blood sugar stabilization.

If someone has problem with high insulin in blood after meal and this insulin is causing reactive hypoglycemia, what should be his diet/supplements?
I am mainly interested in:

• Niacinamide and Aspirin - these 2 supplements should increase use of sugar. In case someone has already problem with low blood sugar, can these 2 supplements cause more problems? Since Niacinamide and Aspirin lower energy supply from fat, how important is it to have steady supply of sugar?

• If someone is for example crashing after orange/apple juice, would you still continue with Niacinamide/Aspirin and sugar from juice? Or is it important to deliver sugar together with some protein like milk/gelatin?

• Does Ray Peat recommend any supplements for blood sugar stabilization like chromium, ALA, vanadium, magnesium....?

• Vitamin B1 helps body metabolize carbohydrates and lipids. I know that this supplement is good for someone with insuline resistance. But is it also good for someone with hypoglycemia?

I am quite surprised that Apple juice is causing blood sugar problem.
Depending on cultivar Apple juice has about 70 percent fructose and
fructose does not need insulin to be metabolised and it helps to
metabolise glucose. It acts a lot like insulin.Apple juice has low
glycemic index and should have low insulin index too. Is it pure apple
juice or apple drink? Store bought apples are usually underripe with a lot of starch.
That kind of apple juice is not going to help blood sugar. I have found big
improvement in maintaining steady blood sugar with Apple juice.

RP recommends Aspirin after meal for maintaining steady blood sugar.
You will need to use vitamin K if you are taking Aspirin regularly.
Niacinamide is especially helpful in fixing liver. Ability to store
glycogen greatly helps with maintaining steady blood sugar.
I think until metabolism is improved it is better to use one of
these free fatty acid inhibitor. You can use small amount of Niacinamide
and Aspirin.

Potassium plays a big role metabolising sugar. Carbohydrate should be
eaten with good amount of potassium. anything that improves sugar
metabolism should fix hypoglycemia. Eating too much protein without
adequate carbohydrate can also increase insulin and cause hypoglycemia.

Total nutritional status and thyroid function is another major cause.
Low thyroid function lowers liver's ability to store glycogen and low selenium
is another common cause. B vitamins, especially B1 and B2 are necessary
for good liver function.

Endotoxin is major cause for all sorts of disease. Regular raw carrot salad
or cooked bamboo shoot should help here. He also recommends casacar, occasional
activated charcoal and low dose antibiotic of certain kind.

If there is a large amount of PUFA storage, one of the FFA inhibitor is almost
essential to protect cells from PUFA damage ,including inhibition of glucose metabolism.
Have you tested your thyroid numbers? Do you track your pulse and temperature?

Thyroid lowers carbohydrate metabolism. You need to make sure you
are getting all the required nutrients. Cronometer.com is very helpful there.

RP has mentioned people can have diabetes from B1 deficiency.

thank you very much for your answer.

Regarding the apple juice, it is very interesting. Would you preffer apple juice to orange juice in this case?

It seems that liver plays important role in glycogen storage.
Is niacinamide detoxifing the liver or correcting the liver cells?
Do you think that liver detox (flushes or herbs) could provide same help with liver?

I do not drink orange juice due to pectin problem and RP recommends
sweet ripe oranges, which are hard to find. For me, apple juice works
way better than OJ in keeping blood sugar steady.
Easiest way is to experiment with both. It is an easy experiment.
Since we do not know what kind of apples are used in juice making
and they often add spoiled apple, you have to
experiment with different brands to find out the suitable one.
Spoiled apple leaves fungal content in the juice.

Niacinamide protects all kind of cell by inhibiting PUFA release.
But i do not know if it has other detoxing effect. My guess is that
if cells are functioning properly then liver will detox itself.
Coffee stimulates liver powerfully. Excess iron is another cause
of liver problem and diabetes. You need to check both ferritin
and Transferrin saturation index to find out your iron storage.
There are studies that showed donating blood improved diabetic condition
and low iron diet improved hepatitis.

I used to do a lot of herbs but stopped all those after
peating. I know that most bitter herbs stimulate liver, coffee is bitter too.
Lowering gut irritation and endotoxin lowers burden on liver and improves
liver function. Most liver herbs probably lowers endotoxin too.
Endotoxin and gut irritation are the root cause of most chronic
diseases. Avoid hard to digest food.Soluble fiber and starch increases endotoxin.

Hi Mittir,

You've been absolutelly right regarding the apple juice. Apple juice is not causing the sugar drop or energy crash.
I had a discussion with friend of mine and he mentioned following example:

At 7 am he ate one slice of white bread with goat cheese with little bit salt. He had no sugar issue at all.
Between 7 - 10 am he was sipping only apple juice. No reactions, no sugar issue.
At 10 am he ate again one slice of white bread with goat cheese and during 1 minute he was shaking, cold and with anxiety.

I am trying to understand what could happened. I know that white bread is very high in glycemic index and probably caused big insulin release to body.
This insulin consumed all sugar from blood and hypoglycemic reaction occured.
But I don't understand why it happened only at 10am.
At 7 am he had absolutely no problem with this food.
After whole night of starving I would expect that body will more likely react with hypoglycemia in the morning than at 10 am. Maybe the morning high cortisol/adrenalin plays some role ?
From 7 till 10 am he was just sipping juice. I would expect stable sugar during this time. I am not sure why this reaction occurred at 10am.

Any idea would be very welcome.

Was it the same amount of bread and cheese? was it a whey based cheese
or casein based cheese? Casein is absorbed slowly and whey is quickly absorbed
Protein is also a major insulin stimulant.
He probably ate less carb with cheese to prevent low sugar.
Blood sugar and insulin has a 2 hour cycle. Body initially start releasing
insulin to clear out glucose and after 2 hours there is another
release of insulin to clear out extra sugar and that is when most
hypoglycemia occurs. I do not know if sipping on sugary drinks is suitable or not.

After waking up body is almost at resting state and with increasing
activity, even staying awake, starts burning energy. Body temperature
and metabolism increases with day light. I drink a cup of AJ
after waking up and then after 1 hour 1 cup of milk with instant coffee
and 2 tbs of sugar. It keeps my blood sugar steady.

10 AM is an adrenaline rush. It happens to me if I have coffee w/o enough food/carbs. For me I have to take 1 tbs sugar when it happens.
" Low blood sugar times are most likely to occur at around 10:00 a.m., 2:00 p.m., and from 3-4:00 p.m."

and for me, when I am at my worst, it occurs at 11:30pm-12:00am and again at 3:00 am during the night
