Activation Of The Endotoxin Receptor (TLR4) May Cause Hearing Loss

There is currently an epidemic of tinnitus (ear ringing) and hearing loss in virtually all developed countries. Experts are scratching heads and are failing to come up with a reason/explanation for this epidemic. As it often happens, the cause is often hidden in plain sight and if one follows the clues and the available evidence, instead of the lofty speeches of medical "experts", the truth gradually becomes clear. A few weeks ago I did a post on serotonin being able to cause tinnitus, which can (logically) be treated by administering a serotonin antagonist. Considering that more than 90% of serotonin is produced in the gut (as a result of endotoxin (LPS) activating the TLR4 receptor), it seems natural to conclude that endoxotin (LPS) would be implicated in hearing loss. Well, as obvious as that may seem, that link had not been established so far. The study below is now the first to prove that activation of TLR4 leads to hearing loss, and as such suggests that reducing endotoxin with antibiotics or (non-soluble) fibrous foods, or blocking its effects with TLR4 antagonists may be a viable treatment for hearing problems. It is worth keeping in mind that endotoxin (LPS) is not the only substance that can activate TLR4. Other TLR4 agonists include PUFA, alcohol (ethanol), opioids, marijuana, SSRI drugs, anticonvulsants, etc. Considering the widespread consumption/use of these substances, it is little wonder that there is an epidemic of hearing loss and tinnitus in the "developed" world. As far as TLR4 blockers, there is no need to wait for the group below to synthesize one. Drugs like cyproheptadine, ketotifen, naltrexone, Benadryl (diphenhydramine), vitamin D/A, emodin, etc are known TLR4 antagonists, and widely available either from a doctor or as a dietary supplement.

Toll-like receptor 4 is activated by platinum and contributes to cisplatin-induced ototoxicity
https://medicalxpress.com/news/2020-08-immune-receptor-thought-linked-loss.html

"..."If we're able make cisplatin, an otherwise invaluable chemotherapeutic tool, safe for everyone—not just low-risk patients—it could be a game-changer for the quality of life of the children using cisplatin as their cancer therapy," said Babolmorad. The platinum atom at the center of cisplatin is responsible for killing cancer cells, but can also activate an immune receptor in the cell membrane, known as TLR4, that experts have long thought plays a role in inflammation. Immune receptors are proteins that detect the presence of a pathogen—or something wrong, like a cancer cell—and generate a signal to combat the pathogen. The inflammation caused by the immune receptor that Babolmorad and her supervisor Amit Bhavsar were interested in occurs in the cochlea and results in damage to the auditory cells and, ultimately, hearing loss. To better understand the mechanism behind this inflammation, th team treated two groups of auditory hair cell lines—similar to the ones found in human ears responsible for perceiving sound—that were essentially identical, except one of them lacked the TLR4 immune receptor, with platinum. "As expected, we only saw signals arise in the cells with TLR4," said Babolmorad. "Basically, platinum, and cisplatin by extension, can activate the TLR4 immune receptor, which causes a response in the immune system—like inflammation in the cochlea." However, thanks to genetic variation, there are people who express a low level of this immune receptor, which means their immune system isn't activated and they have a low risk of developing hearing loss, said Babolmorad. Babolmorad is now working with other U of A researchers, including groups led by chemistry professor Fred West and biology professor Ted Allison, to examine potential synthetic inhibitors that can block the TLR4 receptors."

This is really interesting @haidut.

Recently I had an endotoxin flare up and started getting a weird ear sound along with crazy neurological mania where I couldn't sleep for 48 hours basically. Never happened before.

After a large raw carrot it cleared it up instantly and sleep resumed immediately through an unbelieveable 20 minute nap followed by a full 8 hours that night; and, the mania was gone.

Thanks for sharing the article.

Tinnitus was one of my symptoms (of many) when I decided to really get into health improvements.

It became much softer and softer along the way, up until a point I needed to actively close my ears to hear it. Happy to say, after just checking it, that it is really totally gone!

@haidut What are your thoughts on taurine and tudca, for protecting against LPS?

Besides that, I also think that cellulose powder and wheat bran fibers helps me against endotoxin toxicity. Actually when I am in a place with no good food, whole wheat sourdough bread, with coke and organic cheese is my strategy to get me out of endotoxin misery from bad food.

Have you tried charcoal? For me, it abates endotoxin symptoms within minutes if taken on a empty stomach.

They can increase bile acid synthesis/secretion and that can irritate the gut in some people. I'd say it depends on the dosage. Would definitely not take in the doses that bodybuilder use with the hope of protecting their livers from "juice" side effects.

I've tried it when having flu symptoms but not during an incident similar to the one I described. I will start carrying it with me to have on call.

Are there some charcoal dosages that have worked well for you?

I've had tinnitus since January of 2009. I've tried many many measures to lower it. Any suggestions?

Does alcohol activate TLR4? I've started having problems tolerating alcohol since my health issues started (actually that was the first symptom even when I felt healthy while sober).
For some time now I've had a sensation of blood pumping in my left ear, which started after spending a day outside when it was still quite cold.
I've had some beers last Tuesday (the weather was also pretty bad) and got a cold afterwards, my right ear got blocked and my hearing is around 50% worse than before.. that sucks. Not sure what can be done about this now.. apparently this may be permanent and the only course of action to reverse it are steroids like prednisone. Depends what the underlying issue is, but there is something called 'Sudden Hearing Loss'.

Do you feel pressure / fullness in your ear, does your ear feel physically obstructed? If yes, it is most likely a sinus issue and it is very reversible. Sinus issues might be triggered by cold, it is actually very common.

Is your liver healthy?

What are the best Ray Peat friendly supplements and drugs that can lower endotoxins and endotoxin receptor (TLR4)?

I've been using carrot salad and mushrooms almost every day since I started Peating. For almost a year now.

I've heard @haidut talks about charcoal and I will try it but what about other things? Thanks.

Activated charcoal always works a treat for me. If I feel a bit bloaty or grumbling stomach I'll have some before bed and it sorts it out.

Could you tell me what kind of charcoal are you using? Maybe give me the name of the brand or do you know if Georgie spoke favorably about any specific kind?

This is the one I use (UK). It’s pretty standard I think. I just went for one without any excipients.

Thanks! The issue did reverse but it did feel like physical obstruction before. I also do think I have liver issues.
