Urgent Help Needed, I Believe My Life Is In Danger

TLDR @ BOTTOM

For the past couple months I had been dealing with a lot of constant inescapable stress and trying to follow almost no-fat diet. I was living off of powdered nonfat milk, apple juice, sugar, coffee, cokes, occasional liver, potatoes, and not much else. As time progressed I began to have constantly sweaty palms and feet and got a strange taste in my mouth when I ate carbs. It also seemed like my glycogen storage capacity was decreasing because I seemed to constantly eat(drink apple juice) to feel ok. I thought maybe I had a zinc deficiency so I would occasionally take some zinc gluconate when the symptoms arose. This seemed to help, but only temporarily, and the symptoms would soon return.

About two weeks ago, concerned that I was not getting enough calories, I foolishly mixed 300 grams of organic cane sugar into my milk and gelatin and drank it. About two hours later I started to feel very ill and light-headed. I sat down on the recliner and started to feel like I was going to black out. I was cold and clammy and my heart was going a hundred miles a minute. I persisted in this state for about three hours, intermittently breathing into a paper bag. I downed three or four tablespoons of coconut oil because I thought it would work as fuel if carbs wouldn't. After three hours the tremors had stopped and I felt a bit better. I had the urge to use the bathroom and I ended up eliminate three very large stools over the course of the next couple hours. A taste of lactic acid was in my mouth and my legs burned like I had just run a marathon.

When I tried to drink some carbs again (apple juice or coke I don't remember) I experienced the same lactic acid burn and some vomiting. I decided to try goat milk and cokes with coconut water and mineral water. I still experienced the lactic acid burn and extreme restless legs along with vomiting. The only relief to these symptoms was raw carrot, but then even the raw carrot stopped working and stopped showing up undigested in the stool. I started to experience constant constipation which I could only relieve with some cascara and a saline enema. The symptoms continued and I got very little sleep (maybe an hour a day). This was the first 3 days or so.

I assumed I had become diabetic due to my lack of carbohydrate cofactors and decided to try and correct it with what I had seen Peat say in his interviews. I started taking thiamine and niacinamide and aspirin. They would alleviate symptoms for an hour or so but nothing stuck. At this point I was still drinking cokes, goat milk, mineral water, coconut water and eating coconut oil as my only nutrition. I decided to try a carbonic anhydrase inhibitor so I started taking some acetazolamide I had on hand. The first time I did this I was able to sleep and woke up thinking I was better. This was day 5 or so.

It did not last, as the days progressed I continued to take acetazolamide, bag breathe and take thiamine, niacinaminde, glycine and aspirin. I continued to have the lactic acid symptoms and endotoxic symptoms (low back pain, restless legs etc). Constipation alternated with diarrhea that smelled acidic. I continued to only get an hour of sleep a day, waking up suddenly in a cold sweat. Since raw carrots did not work anymore, I thought mushrooms and bamboo shoots would work. Sometimes they did, sometimes not, so I stopped all the fibers and just took cascara and enemas. Vitamin K2 and D seemed to offer some relief, but again, it was short lived. Oysters seemed to help as well but not for long. I continued to have smaller shaking/blacking out/cold sweat episodes but less severe than the first one.

I decided that maybe the milk lactose was somehow being turned into lactic acid so I stopped the goat milk and only drank cokes and mineral/coconut water ( I know o_O) and ate coconut oil by the spoonful. I still experienced some symptoms but they greatly reduced. Any time I tried meat or anything solid (parmigiano reggiano) I would get strong endotoxin symptoms and parts of my food would pass through in my stools (if I could actually go). I continued to have smaller shaking/blacking out/cold sweat episodes but less severe than the first one. This was 9-10 days in.

I added in honey to my foods because I thought it would help store liver glycogen. I started to develop spider veins where my muscles ached with lactic acid burn. My hemorrhoids seemed to have grown distended and my sphincter seemed to have a decline in function. I decided the cascara was contaminated or not even cascara because of the last symptom (hemorrhoids). I got some Pau D'arco and that seemed to actually work. I decided to try drinking OJ and eating oysters very regularly to see if I had acquired deficiencies from the apple juice/nonfat powdered milk diet. The OJ seemed to not give me the lactic acid symptoms but I think it was too acidic as I experienced calcium and magnesium deficiency symptoms. I started making leaf broth and drinking coffee to remedy this but both leaf broth and coffee seemed to give me endotoxin responses. I stopped eating coconut oil and about two days after I had a another almost blackout/tremors episode. When I ate some honey to restore blood sugar, I experienced more lactic acid symptoms. This was about a week and a half in.

I started eating coconut oil again because it seemed like I had less symptoms when I did. I also switched out the orange juice for clear pulp free apple juice. I also started 1 mg of methylene blue a day. This seemed to help but I couldn't eat any protein without endotoxin symptoms. I finally decided to go to the emergency room. They ran my blood work and it was mostly normal. My CO2 was high and one of my liver values was slightly over the high end of range but the ER doc didn't seem concerned and told me to stop taking aspirin (he ran a salyicates test).

So more apple juice and stopped the coconut oil again (concerned about alfatoxins with my high liver value). Tried to eat some cottage cheese. More endotoxin symptoms. Stringy feces. Tried casein powder, more endotoxin. I decided to try some tetracycline (50mg) and I felt the best I had felt so far. The day of the next dose I didn't take it because I was concered my blood levels of calcium and magnesium were too low from starvation and malabsorption...I didn't want to give myself a heart attack.

Finally I started getting lactic acid symptoms from the apple juice and since I was paranoid of alfatoxins in the coconut oil I didn't take some even though I knew it would help. After thinking about the patterns of the lat week and a half I realized I might have overgrowth of bacteria into my small intestine. I finally decided I would have to use a finely grated carrot as an emergency antibiotic so that I could then drink more apple juice or I would faint. I was going to wait two hours after eating the carrot with vinegar and then try more juice but I ended up fasting instead. I booked an appointment with a urgent care doc. I started drinking coconut water and seemed to tolerate the small amount of sugars in it. He would have none of this bacterial overgrowth talk and had me take a H. pylori breath test. After I drank the test solution I seem to not tolerate coconut water anymore.

I became very despondent and decided to try a different emergency room. I had my blood drawn and then was sent back to the lobby to wait. While waiting I had the worst almost blackout/ tremors episode ever. I had to take a wheelchair into the ER room. This one was worse than the first and I was basically accused of being a hypochondriac. They did give me a general referral to a health clinic next to the hospital. I followed up with that the next day and the doc gave me a general referral to a GI specialist. I have an appointment for an endoscopy today. These last 3 or four days I have eaten maybe 4 oz of liver and some beet green broth. I tried some potato soup but even the residual carbs in that gave me burning stools and burning legs. I am down almost 15 pounds since the beginning of this ordeal and my mental function is starting to break down. I am trying to at least keep my alkaline minerals up by drinking mineral water and calcium bicarbonate water.


TLDR:

In the beginning, 300g of sugar with gelatin and milk gave me lactic acidosis symptoms and endotoxic shock symptoms


Depending on if I eat coconut oil I can seem to absorb different lengths of sugars without the lactic acid symptoms. Lactose never.

Protein and soluble fiber consumption gives endotoxin symptoms

Carbonic anhydrase inhibitor gives only temporary relief from lactic acid symptoms

There is undigested food in my stools

I had constipation now I have diarrhea

Vitamin K2 and glycine give only temporary relief from endotoxin symptoms

Even the sugars in scallops and coconut water seem to create a lactic acid response

I now have a dry cough and pressure behind my eyes, also occasional erratic heart beats

I am down 15 pounds

I can only sleep when I fast

My CK level from my last ER visit was high

This has been going on for more than two weeks

I have an endoscopy appointment today and I have been to the ER two times.




I'm thinking this all seems like D-lactic acidosis and bouts of endotoxic shock brought on by lactobacilli and others overgrowing my small intestine. Brought on by poor nutrition, stress, and sleeping in a hammock with my feet higher than my head. Coconut oil can seem to keep a portion of my small intestine clear but it does not eliminate the endotoxin symptoms. I've attached a paper on D-lactic acidosis.

I'm concerned that I will get to a point where I will be so nutrient and calorie deprived that I won't be able to recover my small intestine even after a course of antibiotics. Again I'm getting an endoscopy today but I have no idea how long it will take to get a doc to test for Small Intestinal Bacterial Overgrowth and also understand the d-lactic acidosis issue.

What should I do? I have some tetracycline on hand and I could order some erythromycin but I don't know if I'm too nutrient and calorie deprived to recover the small intestines natural defenses at this point without a medical administered elemental diet. I also probably have low stomach acid.

I'd love to just hand myself over to treatment of a doctor but that is proving harder to do than I thought. We will see if I can talk to someone willing to test for SIBO at my endoscopy today.

A few general thoughts :

1. Potato juice is well-documented here to solve digestive issues - also, at this point, cutting out starch completely until you have healed would probably be wise (a member named VisionOfStrength wrote a protocol for serious digestive issues that was almost entirely potato juice in the short term)

2. I would replace the sugar with honey just to be safe if your liver is compromised

3. Activated charcoal (1-4 T) a few times might help if you're having a serious endotoxin issue - maybe once every few days (at night) to be safe with plenty of carbs to protect blood sugar

4. Consider that you might be stuck in a vicious cycle of stress, and breaking it with high-fat calorically-dense food (ice cream- HD or RP style) might be necessary in the short term (re-feeding) to get you back to normal before moving forward

5. Caffeine + K2 protocol to clean out your liver might be the best way forward

6. Adding blended raw (or cooked) liver with soda is a easy way to get high-dose vitamins (blend 1 ounce liver to soda or coffee each day) that's easy on the stomach

A few general thoughts too....

You seem to have taken advice from the most common food that Peat speaks about, and reduce your diet to them, but in a very irregular fashion, too much of something at a time. Body's metabolism is so complex that you cannot turn buttons on and off with taking this or that!
The best example is taking 300g of sugar, when our blood only has the equivalent of a spoonful at a time.
Some people also need to increase coconut oil very slowly, to get used to it.
Liver is good but seemed your only animal food.
Whatever your diet before, it is best to change slowly.

Even if you are not hypochondriac, you are similar as a result, your system is stressed, you are afraid. A hypochondriac person is afraid, that is the same.

I got ill last summer, mainly a cold with fungi allergy, after a lot of exhaution.
Same as you I could not sleep, felt I was getting worse and feeling in a real death spiral. Also, I could not even drink more tan a few drops, though I felt dehydrated.

1st good news, I AM ALIVE.
1st of all, trust that your body takes care by itself, you do have to calculate as you do, as you cannot calculate all.
Your body does its best, trust it.
I was really ill, but this attack on my body had stress consequences of course.
So I was lucky enough to get a Somatic Experiencing session. In 1 hour I went from a death spiral to a life spiral. Because of unseen fungi exposure, it took me 1 month to recover, and I got very bad and thin.

But I always know when I feel bad, that this is like clouds hiding the sun.
SUN IS STILL THERE.
This helps to stand the pain.
I am sure you know that we all feel better when we know that the bad is nearly finished.
Get into contact with your inner wise, and know it is there all the time, though at the same time as pain.

At the moment this is the best, as we do not see you. Let doctors take care.
Also tell them that you are not hypocondriac but that you are just scared, and that you know it.
And they have to take it into account, as it does not help you to heal! Hope to get some help that do not include medicine for this part of stress management... Ask what is possible.

Self medicate less, who knows enough, to do it safely? If you are deprived and have weak digestion, they will put you on intravenous feed. Regulate the basics, let them do it, you have time to get to something more perfect after.

You do not tell your age and what person you are, guess male, nor tell how you were before your experiment!

Peat says saturated fat helps with carbs, it is protective. Minimize PUFA does not mean fat free! You probably started to have hypo and hyper glycemia with this diet, and very liquid diet, and no minerals from greens. And probably too Little protein.
Fat and vegetable fiber protects from hyperglycemia. Whole fruit also have fiber. Apple could have been too much fructose for YOUR case.
And if gut is not good, bugs started to thrive on this high sugar.
Your ending disclamer is very honest, bravo, and hold on and get help for balance recovery.

I would ditch the coconut oil in favour of butter and cream. It changed my life. I lived low fat for years and years and higher fat on coconut oil only and my health took dramatic turns for the worse. I had hypoglycemia, hormonal imbalances, extreme fatigue and felt close to suicide at times.

It took many years of supplementing and trying different food combinations to try to figure out the problem. Some things would help for a while, maybe a day or two, then would suddenly make me feel worse. Until I ate a whole jar of cashew butter out of frustration and felt immediately better. Ate a trout with potatoes, drowned in butter and felt awesome. Enough fat and I could tolerate coffee. Coconut oil made me feel a little better, but if it was my only fat i would feel stressed and weird. It felt like it drained me - sugar would give me that odd feeling in my mouth too and I developed chronic candidiasis on my tongue and I presume in my gut.

I emailed Ray and he would often suggest trying higher fat foods to combat my negative reactions to seemingly innocent things. I couldn't tolerate thyroid, and he regularly suggested that.

Try to get out of the negative loops by just saying F**k it. I got to the point where I was like, being fat would be better than this. (And fat surprised me and made me feel slimmer, ironically).

I don't know if it supported my blood sugar, helped remove endotoxin or simply gave me fat soluble vitamins, pleasure or digestibility.

It may not help you, but finding where you are the most 'weird' in your diet and trying to be a bit more normal can really help. Three or four square meals a day- protein, sugar and fat. Low starch if that helps you, higher starch if that helps.

Butter and cream saved my life. Protein and sugar remained high.

You had too much sugar, you are very wise and accept to see the mistake.
You react as if allergic, at the moment, compare to being stung by wasps, 1 or many more is not the same!

I know s.o. who cannot take sugar now, and she was saved by meat.
Try, ghee, clarify your butter to remove lactose.
I also not notice that I can forget about coco when I am into my homemade ghee jar!

I have yet to see anyone have a negative reaction to properly prepared potato juice, so that appears to be the safest food given your situation.

Also, as I stated in my post, if there is something in your gut causing these issues, then activated charcoal is probably the best course to get rid of it. Gut bacteria can cause some truly strange and potent reactions, so instead of trying to alter it, probably best just to get rid of it all together and start with a clean slate.

Mops up a lot I guess.

I think there is not any starch in the potatoe juice?
It is taken by people having stomach ulcer, so yes I can believe it can do good.

I'm gradually trying to incorporate it - but I find the diet and gut measures more important, as my liver struggled to store sugar for so long that taking thyroid in the past seemed to just come to nothing, except being a bit warmer. My pulse was in the 50s when I felt severely endotoxic, however, so clearly it was affecting my thyroid function. I was always caught between "is my thyroid function causing my bad guts" or "is my gut causing my bad thyroid function". It is really both, a vicious cycle, coupled with the deranged hormone pattern that happens over years of severe gut stress. At the moment, I take a little T3 three times or so a day, and 1/8 T3/T4 at night. My TSH at my worst was 1.9, so not that bad, but my T4 was at the bottom of the range. I am hoping that if I can increase my T3/T4 dose without negative symptoms then I can escape the cycle for good. I think for me it all came down to bacterial overgrowth high up in the intestines and the stomach, so that even sugar starts to feed the bacteria, when it is usually the starches. Which is why fatty foods felt kinder on my stomach. Milk invariably becomes difficult until the stomach has a chance to heal and produce sufficient lactase, and limit the negative effects of the bacterial load.

Have you tried antibiotics? They helped me the most when my stomach wasn't tolerating anything, and they got my stomach moving. Erthyromycin and azithromycin helped me, which powerfully stimulate gastric motility, whereas minocycline did nothing. I still use erythromycin daily, a carrot salad (now with coconut oil, which is feeling good now), bamboo shoots, strictly no starch (except for some masa chocolate chip cookies made with coconut oil that seem ok), protein foods fried in butter, coffee, cheese and cooked fruit with cream.

Things are gradually getting better. I notice so many more good days. I have ruined progress by introducing too much milk too quickly when I got cocky and felt great. So I plan to feel good for many months before reintroducing more than a half a cup a day.

Thanks for sharing your experience. Mine is very similar to yours.

For now I take Levoxyl 50mcg at 6 am and 25mcg at 10 Pm twice a week take 50 mcg in instead of the 25.
1/4 grain of Nature Throid twice a week on days that I take 25mcg of Levoxyl at 10 Pm this seems to help me sleep better.

Like you I have gut issues that I think impacts on the absorption of the thyroid medication. Perhaps I should try the T4 and T3 from Haidut as the topicals may work better am not sure.

I have not tried antibiotics yet. Tried Betaine HCL but seems to work for a while and then not at all. Digestive enzymes do not seem to do much for me either. I tried the Peat diet which was great to have and made use of the program of Tom Brimeyer - a very organized program based on the Ray Peat principles. All the dairy, Orange Juice etc caused me severe stomach problems and am still trying to get rid of the bacterial overgrowth caused. Now am still using Peat ideas but let go of the sugar, OJ and dairy. Am trying cooked berries instead of oranges, will try and see what cream will do. II use coconut oil and olive oil but coconut does not always make me feel that well. I shifted using ghee more and limit the coconut and olive oil. My biggest issue is the constant shakiness I have some times it feels as if my whole body trembles.

According to the Dutch adrenal test am low in cortisol bar at night. I just tried adding Vitamin A to my Vitamin D and it caused havoc. My body temperature dropped, I became very dizzy and felt disorientated. Ii took 1000 IU Vitamin A and 2000 IU of Vitamin D both emulsion vitamins as drops. Magnesium makes me even worse as it also lowers my cortisol.

It is all rather confusing and at times am not sure what to eat and what not to have.

Perhaps I can slowly try adding cream and see what happens. Which brand of cream though as am told that some forms of cream and milk may contain gluten since milk is not milk as we used to know it. Some are milk and powder blend that may contain soy or gluten and some creams add thickening agents.

Eat fatty red meat, egg yolks, liver, bone and leaf broths for a few days. you've depleted your nutrient stores and you're catabolic. Sugar is only protective when sufficient nutrients are present. If they are not, you're literally tearing yourself apart forcing down more sugar. Taking isolated supplements is damaging you more as well.

Thanks sooo much. Leaf broths. What do I use and how are they made? Should I drop all supplements for now?
